I am writing to request you a leave of absence for about two week starting from 21st May, 2009 for the reason given in this letter. My mother has been diagnosed as having a cyst and needs to undergo surgery for the same. She needs someone to accompany her to the hospital for the surgery and also needs someone to be with her during recovery. The entire process, according to the doctor, will take about two weeks. As my mother and I are the only ones in the family, it is obvious that this responsibility will fall on my shoulders.
Hence I request you to grant me leave for a period of one week starting 21st May. I shall resume work on 28th May, 2009 but if there is any problem, I shall give you prior notice of delay.
